Outline
Financial eligibility guidelines
The process is designed to confirm financial eligibility without asking applicants to disclose sensitive personal information.
❋ STEP 1. Sign the Declaration Form
Confirm that the regular cost of private Pilates is a genuine financial barrier and that the information provided is truthful.
❋ STEP 2. Submit one supporting document
Choose ‘Route A.’ for current government assistance or ‘Route B.’ for income-based eligibility using page 1 of Tax Form 1040.*
(*) Applicants only need to qualify through one route. Eligibility does not guarantee immediate placement.

Government-assistance participation
Other income-based assistance programs may be reviewed individually, but submission does not guarantee eligibility.
Qualifying programs
Submit one current eligibility, approval, renewal, or a screenshot from the official program portal.

Eligibility Route (B)
Income-based Eligibility
Provide evidence of your current adjusted gross income on Tax Form 1040 line 11(a).
Applicants who were not required to file a federal tax return may request an individualized eligibility review.
Qualifying AGI Income Chart
Submit Page one (1) of your most recently filed Tax Form 1040 or 1040-SR. Adjusted gross income shown on Line 11(a) should be below the following thresholds:

All Social Security numbers, addresses, dependent details, and other sensitive information.
Applicant name, filing status, household size, and adjusted gross income.
NYC public-accommodation protections prohibit unequal treatment based on actual or perceived immigration status or national origin. Align Studio does not use immigration-related inquiries as part of Community Program eligibility.
New York’s SHIELD Act requires businesses maintaining private information to use reasonable administrative, technical, and physical safeguards and to dispose of private information within a reasonable time after it is no longer needed. Align Studio does not store or share your private information.

If you're admitted, you'll receive a confirmation by e-mail. Each time a new week's schedule is released, you'll receive a private booking link to reserve your session.

One (1) supporting document for your chosen eligibility route.
Applications are considered complete only after all required supporting documentation has been received.

No. Program capacity is limited, approved applicants may be placed on a waitlist when all subsidized spaces are filled.

Align Studio should give the applicant a simple opportunity to correct or replace the document before denying the application.

Eligibility may remain valid for 6 months, followed by a simple annual renewal with a new declaration and current supporting document.

Yes. Other income-based government programs may be reviewed individually, but they do not automatically guarantee eligibility.

Yes. Applicants who were not required to file, or whose current circumstances materially changed, may request individualized review.
